Dear Parents -
In recent months we have all become aware of the serious medical issues that are developing around the world. In an attempt to stay ahead of these concerns, Prep has begun to take steps to protect our community. Since our Resident program has such a large international population that will be traveling during spring break, we have decided to begin by monitoring and collecting information on student travel plans. Please assist us by filling out the following travel form for your son and returning it to us as soon as possible. Download PDF Travel Form
We have also included another form that provides new information regarding your son's return to Prep after break. We are asking that each student check in with the Health Office when they return, for a brief evaluation. More information is provided on the form, itself. Georgetown Prep Health Forms
You will find all forms that your son needs to start school for proper health clearance. Every student needs the following forms: *Medical Information Form *Insurance/Health History Form *Physical Exam Form (must be completed by MD) *Immunization Form *HIPPA/Over the Counter Med Form
If your son takes a medication regularly, you must fill out the long-term medication form.
